For use only with the PDF format Course Reserves Request Form. Must be signed and returned to the Library if any copyrighted materials are being placed on reserve. A one-time requirement.

I understand and agree to adhere to the copyright standards established for placing materials in the Paper/AV Reserve Collection and on EReserves as outlined in the Copyright Guidelines. I understand that my failure to do so may result in having items refused for placement in the Reserve Collection or on EReserves.
